HealthRIGHT 360 in California is seeking a dedicated Case Manager to coordinate care across outpatient, residential, or community settings for individuals with substance use disorders and co-occurring mental health conditions. The role emphasizes intake assessment, benefits linkage, and collaboration with a multidisciplinary team.

You will support clients in applying for Medi‑Cal, securing housing options, and connecting with health and social services.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in Social Work, Psychology, or related field preferred or required.
• Substance Use Disorder Counselor registration from a California agency (CCAPP, CAADE, CADTP) required or preferred.

Responsabilités

• Assist client intake by completing case management assessment and entering financial and benefit information.
• Help clients apply for Medi‑Cal or transfer Medi‑Cal County when appropriate.
• Assess client needs and complete releases of information.
• Connect clients to benefits, mental health, physical health, employment, housing, and community resources.
• Coordinate communication with probation, DCFS, and other agencies; schedule appointments and reminders.
