Job summary

HealthRIGHT 360 is seeking a Case Manager II to support inmates in Solano County with comprehensive, person-centered re-entry planning and coordinated care.

You will develop personalized Re-entry Plans, connect participants to housing, employment, benefits, and services, and maintain ongoing care with a holistic, client-centered approach. The role is hybrid on-site and field-based, requiring strong collaboration with housing navigators and community partners.

Qualifications

• High School Diploma or GED + AOD Registration/Certification.
• Bachelor's Degree + 2 years Case Management experience.
• Willingness to work a hybrid on-site (70%) and field-based role (30%).

Responsibilities

• Conduct intakes and assessments for ECM participants.
• Develop and track individualized Re-entry Plans with housing and resources.
• Coordinate housing, employment, benefits, and legal services.
• Meet participants every two weeks and document progress.
• Collaborate with housing navigators and community partners.
• Document interactions in Welligent within 24 hours.
• Coordinate care across behavioral health providers.
• Assist with transitions from jail to community-based care.
