HealthRIGHT 360, through North County Serenity House, seeks a Case Manager I to coordinate care across outpatient, residential, and community settings. You will assist with intake, assess needs, and help clients apply for Medi-Cal and other benefits.

You will work with a multidisciplinary team to ensure treatment plan goals are met, document progress within 72 hours, and coordinate with probation, DCFS, housing, and employment services.

Qualifications

• Must have strong case management skills and the ability to document accurately.
• Experience coordinating services across outpatient, residential, and community settings.
• Ability to assist with intake, benefits applications (Medi-Cal), and care planning.

Responsabilités

• Assist the client with intake by completing case management assessment and entering financial and benefit information.
• Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
• Assess the client’s case management needs and complete all releases of information.
• Connect the client to benefits, mental health, physical health, employment, probation, DCFS, housing, community resources, outpatient substance use disorder services, and aftercare.
• Coordinate communication and external service linkage including scheduling appointments and contacting probation/DCFS.
• Deliver services across outpatient, residential, or field settings as needed.
• Prepare clients for job search with resume writing and preparation.
• Monitor progress toward treatment plan goals and provide input.
• Seek clinical consultation when facing high-risk circumstances.
• Collaborate with clinical staff including therapists, medical, and psychiatry.
