Job summary

Qualifications

• Minimum high school diploma or GED; AOD certification preferred.
• Bachelor's degree plus 2 years case management experience required.
• Experience with EHR systems (e.g., Welligent) is preferred.

Responsibilities

• Conduct intakes and assessments.
• Develop and track individualized Re-Entry Plans.
• Coordinate housing, employment, and public benefits resources.
• Meet clients biweekly post-release and adjust plans as needed.
• Document interactions and maintain care plans in a timely manner.
• Represent HR360 at community outreach events and coordinate with partners.
