Changing Perceptions, Inc. in the Washington, D.C. metropolitan area is seeking a Re-entry Case Manager to support justice‑impacted individuals through intake, ISP development, referrals, and ongoing case management.

Reporting to the Chief Program Officer, you will coordinate care, navigate housing, employment, healthcare, and benefits, maintain documentation, and provide crisis support as needed while collaborating with community partners.

Qualificações

• Experience in case management, social work, re-entry services.
• Committed to dignity and empowerment of justice-involved individuals.
• Highly organized with ability to manage multiple caseloads.
• Strong verbal and written communication; comfortable with one-on-one and group interactions.

Responsabilidades

• Conduct Participant Intake & Assessment.
• Develop Individualized Service Plans (ISPs).
• Provide Case Management & Support via referrals, scheduling, transport, check-ins.
• Facilitate Resource Navigation to benefits, training, legal aid, and more.
• Track Progress & Maintain Documentation per funder requirements.
• Engage in Crisis Intervention and connect to longer-term resources.
• Collaborate with partners and re-entry networks for wraparound support.
• Advocate for participants across criminal justice, housing, employment, and healthcare.
• Support Program Development with feedback on trends.
