Concordance in St. Louis, MO seeks a dedicated Post-Release Case Manager to guide participants through reentry, advocating for their progress and coordinating care teams.

You'll lead 1:1 and group sessions, connect participants to housing, healthcare, and other resources, and document progress in our electronic records. The role emphasizes trauma-informed practice and collaboration with the care team.

Qualifications

• Bachelor’s degree in a social service-related field (social work, psychology, counseling, criminal justice, sociology, or similar).
• CPR/First Aid, preferred, or able to complete within the first 90 days.
• Ability to sit or stand at a computer for typing and reading; comfortable in group and 1:1 sessions.

Responsibilities

• Facilitate 1:1 and group sessions with participants to assess needs and progress.
• Lead life-skills groups on topics like money management and self-care.
• Maintain treatment goals and trauma-informed reflections.
• Document sessions and reflections in the electronic record system.
• Collaborate with the Care Team and participate in meetings and trainings.
