Job summary

Sanctuary Recovery Centers is seeking a Justice-Involved Case Manager (Reentry & Prison Coordination) to support individuals preparing for release and successful reentry into the community across Arizona. This field-based role focuses on coordinating services connected to correctional facilities, probation/parole systems, and reentry programs to ensure continuity of care and compliance with requirements.

Qualifications

• Experience with justice-involved individuals.
• Knowledge of court, probation and parole processes.
• Ability to work independently in field-based environments.
• Strong documentation and caseload management.
• Valid Arizona driver's license and reliable transportation.
• Ability to pass background checks and fingerprint clearance.

Responsibilities

• Maintain communication with correctional facilities, probation, parole, and courts.
• Coordinate reentry services prior to and after release.
• Develop individualized service plans and connect clients to housing, employment, health, and community resources.
• Provide ongoing follow-up and progress monitoring.
• Travel within assigned regions near correctional facilities.
• Represent the organization in justice-system collaboration efforts.
