Job summary

Broad Street Love in Philadelphia is seeking a Reintegration Case Manager to support guests formerly incarcerated. You will assess needs, develop integration plans, and help access housing, employment, education, and healthcare services.

You will collaborate with partners, maintain caseloads, document progress, and advocate for equitable access to resources while supporting reentry planning and community connections to reduce recidivism.

Qualifications

• Bachelor degree in social work, psychology, public health or related fields.
• Four years of experience in case management and social services.
• Experience with unhoused populations.
• Familiarity with local social service systems, housing programs, and workforce development resources.

Responsibilities

• Conduct comprehensive needs assessments for new guests to identify strengths, barriers, and goals.
• Develop individualized service plans tailored to each guest’s needs (housing, employment, education, healthcare).
• Maintain ongoing caseload with case management and follow-up to ensure progress toward integration goals.
• Assist guests in navigating systems such as housing, social services, education, and healthcare; advocate for equitable access.
• Support clients in obtaining legal documentation (IDs, work permits).
• Collaborate with community partners, agencies, and providers to coordinate resources (job placement, entrepreneurship programs).
• Connect guests with job readiness programs, vocational training, and education opportunities.
• Provide mental health crisis intervention to guests in active crisis.
• Travel to correctional facilities for in-reach services and reentry planning.
• Maintain up-to-date case notes, service plans, and progress reports in CM systems.
• Use data systems to collect, track, and report program data for funders and evaluation.
• Ensure compliance with policies, confidentiality, and grant requirements.
• Stay informed about resources and best practices.
• Participate in trainings, staff events, and partner groups.
