Groups Recover Together, Inc. invites applications for a Recovery Support Coordinator to join our care team in supporting 160–180 members with health-related social needs and coordinated care across clinical, social, and community settings.

The role emphasizes compassionate, outreach-based engagement, navigation of services, and collaboration with a multidisciplinary team to improve engagement and recovery outcomes; schedule includes a 4-day workweek with some remote work.

Qualifications

• High school diploma required; related associate or bachelor degree preferred in Tennessee/VA contexts.
• Experience with EHR systems and cross-functional care coordination is desirable.
• Ability to navigate social services, housing, transportation, and insurance.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships and support members in achieving recovery goals.
• Identify member goals, provide coaching, and engage in recovery activities.
• Reengage unengaged members and support transitions to care.
• Provide in-person, virtual, and telephonic support per regional needs.
• Maintain caseloads and coordinate care pathways, including referrals and ROIs.
• Engage in community outreach and promote recovery-positive language.
