Groups is an outpatient provider delivering SUD treatment and recovery support across community and clinical settings. We seek a Recovery Support Coordinator to guide 160–180 members through health-related social needs, engagement, and coordinated care on a hybrid schedule.

You will partner with a care team to coordinate services, advocate for resources, and help members navigate complex systems while maintaining compassionate, recovery‑oriented support.

Formación

• High school diploma required; related degree preferred.
• Two years experience in case management or care coordination.
• Experience with SUD or mental health challenges preferred.
• Experience using Electronic Health Records (EHR).
• Reliable internet access and ability to work remotely if needed.

Responsabilidades

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships with members to support recovery goals.
• Assist members in navigating insurance, housing, transportation, and community resources.
• Manage referrals, releases of information (ROIs), and other documentation for smooth transitions of care.
• Provide in-person, virtual, and telephonic support as needed.
