Job summary

Groups Recover Together is seeking a Recovery Support Coordinator in Ellsworth, ME. The role focuses on supporting members with health-related social needs, coordinating care across clinical and community settings, and driving engagement in recovery services.

You will work with a care team to deliver person-centered services, assist with referrals, and help members navigate insurance and community resources. A hybrid schedule is available, with in-office and remote components as needed.

Qualifications

• Requires high school diploma; associate degree in related field preferred.
• Bachelor’s degree may be required in some states (Virginia/Tennessee).
• MHRT/C or CHW certification may be preferred or required in select programs or states.
• Experience with EHR and care coordination is expected.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure highQuality, member-centered care.
• Build trusted relationships with members to support recovery goals through empathy and motivational interviewing.
• Identify member goals, provide coaching, coping skills, and engagement in recovery activities.
• Support unengaged members to reengage in treatment services and onboard new members.
• Provide in-person, virtual, and telephonic support as needed; maintain shared caseload and coordinate services.
