Job summary

Groups is a leading outpatient provider offering SUD treatment with a focus on recovery support. The Recovery Support Coordinator guides members through their recovery journey, coordinating care across clinical and community settings and helping them access resources and services.

This role involves direct member engagement, goal setting, and outreach to reengage individuals in treatment, with a 4-day workweek and occasional remote Tuesdays. Requires experience in care coordination and EHR usage.

Qualifications

• High school diploma required.
• Associate’s degree in Social Services, Human Services, Psychology, or related field preferred.
• Minimum of two (2) years’ experience in case management, care coordination, peer support, or related field preferred.
• Experience using Electronic Health Records (EHR) required.
• Reliable internet access and strong WiFi if remote work.
• Must meet state background screening and pre-employment requirements.
• Valid driver’s license and access to a vehicle.

Responsibilities

• Provide recovery support and coordinate care across clinical, social, and community settings.
• Assist members in identifying goals and engage in coaching and recovery activities.
• Support engagement in treatment services through outreach and case management.
• Deliver in-person, virtual, and telephonic support based on regional needs.
• Maintain a shared caseload and ensure coordinated service delivery.
