Job summary

Groups Recover Together is seeking a Recovery Support Coordinator to join the care team, managing a panel of 160-180 members with Health-Related Social Needs, treatment engagement, and coordinated care across clinical and community settings.

The role includes a 9a-7p schedule, with WFH on Wednesdays; salary ranges from $45,000 to $47,000. Responsibilities emphasize patient-centered care, outreach, and collaboration with counselors and prescribers.

Qualifications

• High school diploma required; associate’s degree preferred.
• Bachelor’s degree in related field (Virginia and Tennessee).
• Minimum two years’ experience in case management, care coordination, or related field preferred.

Responsibilities

• Collaborate with multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships with members to support recovery goals using empathy and motivational interviewing.
• Assist members with identifying goals and providing coaching and engagement in recovery activities.
• Support disengaged members to reengage in treatment services through outreach.
• Provide in-person, virtual, and telephonic support based on regional needs.
• Maintain a shared caseload and coordinate services to remove barriers to care.
• Navigate insurance, housing, transportation, and other community resources for members.
• Coordinate referrals and documentation to ensure smooth transitions of care.
