Job summary

Groups in Scottsburg, IN is seeking a Recovery Support Coordinator to join the care team. You will provide recovery support services, coordinate care across clinical, social, and community settings, and assist 160–180 members with Health-Related Social Needs (HRSN) throughout their recovery journey.

The role reports to the Recovery Operations Supervisor and offers a hybrid schedule with potential evenings. Strong experience in care coordination and peer support is preferred.

Qualifications

• Strong interpersonal skills and ability to build trust with members.
• Experience in care coordination and multidisciplinary teams.
• Knowledge of recovery-oriented care, motivational interviewing, and trauma-informed practices.
• Experience with Electronic Health Records (EHR).
• Peer support certifications may be preferred or required in some programs.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships to help members achieve recovery goals.
• Assist members in identifying goals and provide coaching and engagement in recovery activities.
• Support unengaged members to reengage in treatment through outreach.
• Coordinate transitions to care, including referrals, releases of information, and documentation.
• Provide in-person, virtual, and telephonic support based on regional needs.
• Maintain a shared caseload and ensure coordinated service delivery.
• Connect members to healthcare, behavioral health, social services, and community resources.
