Job summary

Groups is a leading outpatient provider specializing in substance use disorder treatment. The Recovery Support Coordinator will work directly with members to provide recovery support services and coordinate care across clinical, social, and community settings.

Responsibilities include managing a panel of 160-180 members, assisting with treatment engagement, care coordination, and navigation of health and social services.

Qualifications

• Minimum two years' experience in case management or care coordination.
• Experience working with individuals with substance use or mental health challenges preferred.
• Reliability in using Electronic Health Records (EHR).
• Must meet state-specific background screening and pre-employment requirements.

Responsibilities

• Collaborate with multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships to support recovery goals through empathy and motivational interviewing.
• Identify goals and provide coaching and engagement in recovery activities.
• Support re-engagement of unengaged members through outreach.
• Coordinate transitions using referrals, ROIs, and documentation.
• Deliver outstanding customer service and maintain compliant EHR records.
• Assist with scheduling, group facilitation logistics, and communications.
• Engage in community outreach and education activities.
