Groups Recover Together in Port Richey, FL is seeking a Recovery Support Coordinator to join the care team and help 160–180 members with health-related social needs, treatment engagement, and coordinated care across clinical and community settings. Qualifications

• Salary range: $43,000-$50,000 per year; based on experience and education.
• Minimum of two years’ experience in case management, care coordination, or peer support; experience with SUD/mental health preferred.
• High school diploma required; an associate degree in related field preferred.
• Peer Recovery Coach certification may be preferred or required in select programs or states.
• Experience using Electronic Health Records (EHR).
• Must meet state-specific background screening and pre-employment requirements; Florida info link.
• Valid driver's license and access to a vehicle (if role includes community travel).

• Build trusted relationships to support members' recovery goals.
• Coach members on goals, coping skills, and engagement in recovery activities.
• Re-engage unengaged members through intensive outreach.
• Assist new members through transition to care and outreach.
• Provide in-person, virtual, and telephonic support as needed.
• Maintain a shared caseload and coordinate service delivery.
• Navigate systems like insurance, housing, transportation; connect to resources.
• Manage referrals and releases of information for smooth transitions.
• Promote recovery-positive language and community engagement.
