Qualifications

• High school diploma required; associate or bachelor’s degree preferred in related fields.
• MHRT/C or CHW certification may be required/ preferred in certain programs or states.
• Experience with care coordination, case management, or peer support is preferred.
• Experience using Electronic Health Records (EHR) and strong organizational skills.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships with members to support recovery goals.
• Provide coaching, coping skill support, and engagement in recovery activities.
• Support re-engagement in treatment through outreach and engagement efforts.
• Assist members in navigating insurance, housing, transportation, and social services.
• Coordinate referrals, ROIs, and ensure smooth transitions of care.
• Promote recovery-positive language and connect members to community resources.
