Job summary

Groups seeks a Recovery Support Coordinator in Martinsville, IN (hybrid). You will work with a care team to coordinate services for 160–180 members, linking them to healthcare, housing, transportation, and community resources. The role emphasizes empathy, motivational interviewing, and reducing barriers to care while guiding members toward recovery goals.

Salary range $43,000–$50,000, based on experience. Schedule: Mon–Thu, 4-day workweek with one WFH day; some evenings may be required.

Qualifications

• Bachelor’s degree or equivalent in a related field preferred.
• Experience in case management or care coordination.
• Experience with Electronic Health Records (EHR) and health systems.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships with members to support recovery goals.
• Assist members in identifying goals and engaging in recovery activities.
• Support unengaged members to reengage in treatment services through outreach.
• Provide in-person, virtual, and telephonic support based on regional needs.
• Maintain a shared caseload and coordinate service delivery.
• Link members to healthcare, social services, and community resources to meet needs.
