Groups is a leading outpatient provider delivering recovery support services in Pikeville, KY. The Recovery Support Coordinator works with 160-180 members, coordinating care across clinical and community settings to improve engagement and outcomes.

Compensation ranges from $44,000 to $50,000, and the schedule supports a 4-day workweek with specific evening hours. This role emphasizes patient-centered support, care navigation, and collaboration with a multidisciplinary team to connect members

Qualifications

• High school diploma required; associate/bachelor's preferred depending on state.
• Two years in case management or care coordination preferred.
• Experience with substance use or mental health challenges is a plus.
• Reliable internet access; valid driver’s license and vehicle may be needed.
• State-specific background screening and pre-employment requirements may apply.

Responsabilités

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted member relationships using empathy and motivational interviewing.
• Assist members in identifying goals and engaging in recovery activities.
• Support reengagement for unengaged members and transitions to care.
• Provide in-person, virtual, and telephonic support; maintain a shared caseload.
• Link members to healthcare, behavioral health, and social resources; follow up.
• Navigate insurance, housing, transportation, and other practical systems.
• Manage referrals, ROIs, and documentation to facilitate transitions of care.
