Qualifications

• High school diploma required; associate degree preferred.

• Minimum of two years’ experience in case management or care coordination.
• Experience working with individuals with substance use or mental health challenges preferred.
• Experience using EHR systems.
• Reliable internet access and strong WiFi when working remote.
• Must meet state background screening and pre-employment requirements.
• Valid driver’s license and access to a vehicle for community travel.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high‑quality, member‑centered care.
• Build trusted relationships to support recovery goals through empathy and motivational interviewing.
• Identify goals and provide coaching, coping skill support, and recovery activities.
• Reengage unengaged members via intensive outreach.
• Introduce Groups’ care model to new members and support transitions to care.
• Provide in-person, virtual, and telephonic support as needed.
• Maintain a shared caseload and coordinate service delivery.
• Link members to healthcare, social services, and community resources.
• Navigate insurance, housing, transportation, and other supports.
• Manage referrals and releases of information for smooth transitions.
• Engage in community outreach and act as a recovery advocate.
