Qualifications

• High school diploma required; associate's degree preferred.
• Experience with recovery-support, case management, or care coordination preferred.
• Experience using Electronic Health Records (EHR) is preferred.

Responsibilities

• Provide recovery support services and care coordination across clinical, social, and community settings.
• Assist members with navigating insurance, housing, transportation, and community resources.
• Manage referrals and care documentation to ensure smooth transitions.
• Engage members with outreach to reengage them in treatment and support recovery goals.
• Deliver in-person, telephonic, and virtual support based on regional needs.
• Collaborate with a multidisciplinary care team to deliver high-quality, member-centered care.
