Qualifications

Responsibilities

• Provide recovery support and care coordination with a multidisciplinary team.
• Assist members in navigating social services, housing, transportation.
• Deliver in-person, virtual, and telephonic support.
• Maintain documentation in EHR and manage caseload.
• Engage in community outreach and education.
• Participate in interdisciplinary team meetings.
