Job summary

HealthRIGHT 360 North County Serenity House is seeking a dedicated Case Manager I to coordinate care across outpatient, residential, and community settings. You will assist with intake, complete assessments, and help clients access Medi-Cal benefits and housing resources.

Collaborating with a multidisciplinary team, you will document progress, support treatment planning, and ensure compliance with HIPAA and relevant standards while transporting clients as needed.

Qualifications

• Case management experience.
• Experience working with clients experiencing acute withdrawal from substances.
• Experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.
• High school diploma or equivalent; SUD Counselor registration required per option I/II.

Responsibilities

• Assist the client with intake by completing case management assessment and entering financial and benefit information.
• Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
• Assess the client’s case management needs and complete all releases of information.
• Connect the client to benefits, mental health, physical health, employment, probation, DCFS, housing, community resources, outpatient substance use disorder services, and aftercare.
• Coordinate communication and external service linkage including scheduling appointments, probation, DCFS, and court documents; provide appointment reminders.
• Deliver services in outpatient, residential, or field settings as needed.
• Prepare clients for job search with resume writing and job search prep.
• Monitor progress toward treatment plan goals and provide input.
• Seek clinical consultation as needed and work with the treatment team.
