Job summary

Qualifications

• High school diploma or equivalent.
• Substance Use Disorder Counselor registration from a California agency.
• Bachelor’s degree in Social Work, Psychology, or related field.
• Case management experience in human services.
• Valid First Aid and CPR certification.

Responsibilities

• Assist intake by completing case management assessment and entering financial information.
• Support Medi-Cal enrollment and transfers as appropriate.
• Assess client needs and manage releases of information.
• Connect clients to benefits, health services, housing, and employment resources.
• Coordinate scheduling, court orders, and ASL/therapy appointments.
• Provide services in outpatient, residential, or field settings.
• Complete client progress notes within 72 hours; ensure documentation compliance.
• Transport clients as needed and participate in supervision.
