Qualifications

• Bachelor's degree in social work, psychology, counseling, human services, nursing, healthcare administration, or related field.
• 1–2 years of case management, care coordination, or related clinical/human services experience.
• Behavioral health, substance use treatment, or primary care experience preferred.
• Proficiency with EHR systems and clinical documentation.
• Availability: Mon–Fri 3:00–7:00 PM; Sat–Sun 9:00 AM–3:00 PM.
• Valid California driver's license, reliable transportation.

Responsibilities

• Conduct biopsychosocial assessments and develop individualized care plans with the clinical team.
• Serve as the primary contact for clients navigating clinic services and coordinate referrals.
• Connect clients with community resources and assist with insurance benefits and authorizations.

• Serve as the primary point of contact for clients navigating clinic services and coordinate referrals to internal and external providers
• Connect clients and families with community resources (housing, transportation, financial assistance, social services) and assist with insurance benefits and prior authorizations
• Maintain accurate, timely documentation in the EHR and ensure compliance with regulatory and payer requirements
• Participate in clinical team meetings, case reviews, and treatment planning conferences
• Build trust-based relationships with clients and families, educating and empowering them throughout their care

Qualifications

• Bachelor's degree in social work, Psychology, Counseling, Human Services, Nursing, Healthcare Administration, or related field
• 1–2 years of experience in case management, care coordination, or a related clinical/human services role
