Job summary

HealthRIGHT360, a nonprofit organization, seeks a dedicated Case Manager for North County Serenity House in Carlsbad, CA. The role coordinates intake, assessments, and benefits applications, linking clients to Medi-Cal, housing, employment, and health services.

As part of a multidisciplinary team, you’ll ensure documentation compliance, write progress notes, and support recovery and community reintegration.

Qualifications

• High school diploma or equivalent.
• Substance Use Disorder Counselor registration from a California agency (CCAPP, CAADE, CADTP).
• Minimum two years of experience as a job developer or employment specialist.
• Experience teaching adults and/or facilitating workshops.
• Commitment to working with diverse communities, including communities of color and LGBTQ+ individuals.

Responsibilities

• Assist clients with intake, case management assessments, and benefit information entry.
• Support Medi-Cal applications and transfers when appropriate.
• Coordinate connections to benefits, health services, housing, and employment.
• Schedule appointments, communicate with probation and DCFS, ensure court orders are obtained.
• Deliver services across outpatient, residential, or field settings as needed.
• Prepare clients for job search with resume writing and planning.
