Job summary

HealthRIGHT 360, North County Serenity House in Escondido, CA, is seeking an in-person Case Manager to coordinate care across outpatient, residential, and community settings. You will assist with intake, assessments, and benefits applications, while collaborating with a multidisciplinary team to achieve optimal treatment outcomes.

Qualifications

• High school diploma or equivalent.
• SUD Counselor registration from a CA agency (CCAPP/CAADE/CADTP).
• Bachelor’s degree in Social Work or related field preferred.
• Case management experience.
• Valid CA driver’s license and access to transportation.

Responsibilities

• Assist intake by completing case management assessment and entering financial and benefit information.
• Support clients in applying for Medi-Cal or transferring Medi-Cal County as needed.
• Assess client needs and complete all releases of information.
• Connect clients to benefits, health, employment, housing, and community resources.
• Coordinate communication with probation, DCFS, appointments, and aftercare.
• Deliver services in outpatient, residential, or field settings as required.
• Prepare clients for job search with resume writing and job search prep.
• Monitor progress toward treatment plan goals and provide input.
• Collaborate with clinical team and communicate with medical staff.
• Transport clients as needed and complete UA tests.
