Qualifications

• High School diploma required; training or relevant work or lived experience in mental health or addiction preferred.
• Bachelor's degree in psychology, social work or related human services strongly preferred; may be waived for last-semester candidates.
• Experience delivering community- or hospital-based services is a plus.

Responsibilities

• Serve as a subject matter resource regarding local addiction treatment and recovery options.
• Engage ED/inpatient patients seeking detox or treatment and provide education on options.
• Develop relationships with detox/treatment centers to streamline referrals and admissions.
• Coordinate with patients, centers, Recovery Coach, and health team to submit required intake materials.
• Track referrals and follow up with centers and patients to reduce barriers and ensure transitions.
• Provide follow-up outreach after discharge and connect to peer recovery supports and community resources.
