Gosnold, Inc. is seeking an ED Navigator for 40 hours at Stoughton, MA, to assist patients in the emergency department who have substance use disorders. You will engage patients and families, coordinate referrals, and support transitions to treatment with hospital staff.

The role requires education in counseling or related field and prior experience in behavioral health, with a focus on compassionate, non-judgmental communication and strong collaboration with clinical teams.

Formación

• Bachelor’s Degree in a counseling related discipline or 1 year of experience in related field.
• At least two years working with substance use disorder patients in a behavioral health clinical setting; three years of lived addiction recovery experience may substitute for workplace experience.
• Ability to interact and relate to persons experiencing an acute drug or alcohol crisis in a non-judgmental manner; communicate hope, respect and empowerment.
• Willingness to undergo certification training with a recognized Coaching Certification organization.

Responsabilidades

• Engage with ER patients with substance use disorders in a non-judgmental manner to refer to addiction-specific services.
• Conduct screening assessments with patients directed by the ER physician to ascertain suitability for referrals.
• Provide recovery coaching and mentoring to maximize referral placement.
• Facilitate transfer to treatment settings and coordinate transportation as needed.
• Develop referral relationships with community resources across a wide geographic area.
• Conduct follow-up calls to patients who refuse treatment referrals.
• Document in the hospital record-keeping system while maintaining privacy and confidentiality.
