Qualifications

• High School diploma required.
• Training or relevant work or lived experience in mental health and/or addiction/recovery preferred.
• Bachelor’s Degree in psychology, social work or related human services field strongly preferred.
• Certificates, Licenses, Registrations (e.g., LADC/CADC) preferred.

Responsibilities

• Serve as a subject matter resource regarding addiction treatment and recovery options.
• Engage ED patients seeking detoxification and educate on treatment pathways.
• Coordinate referrals and admissions with treatment centers and Recovery Coach.
• Track referrals and follow up to reduce barriers to care.
• Document interactions and care coordination in Epic with confidentiality.
