Beverly Hospital and Addison Gilbert Hospital seek a part-time Recovery Support Navigator to assist individuals with substance use-related needs in the ED and inpatient settings. You will guide patients through treatment and recovery pathways, coordinate with treating teams, and connect them to detox, treatment, and community resources.

Responsibilities include documenting interactions in Epic, following up after discharge, and facilitating smooth transitions to care with a focus on harm

Formación

• High School diploma is required or preferred depending on role.
• Training or relevant work or lived experience in the areas of mental health and/or addiction/recovery preferred.
• Bachelor's Degree in psychology, social work or related human services field is strongly preferred; may be accepted in last semester under waiver.

Responsabilidades

• Serve as a subject matter resource regarding the local addiction treatment and recovery support system, including pathways to detoxification, residential, outpatient care, MAT, and community resources.
• Engage individuals in the ED seeking detoxification or other treatment and educate on options.
• Develop and maintain collaborative relationships with detoxification and treatment centers to streamline referrals and admissions workflows.
• Identify available detoxification and treatment beds based on patient needs, preferences, and eligibility.
• Coordinate with patient, Recovery Coach, and healthcare team to obtain and submit required documentation for referrals and admissions.
• Track referrals and follow up with centers and patients to reduce barriers and support transitions to care.
• Provide follow-up outreach after ED or inpatient discharge to assess engagement with treatment and recovery supports.
• Identify individuals who may benefit from continued engagement with the Recovery Coach or peer recovery services after discharge.
• Provide linkage to peer recovery networks, mutual aid groups, and community resources when appropriate.
• Collaborate with the Addiction Consult Service Recovery Coach to ensure continuity of support and avoid duplication of services.
• Document interactions, referrals, outreach efforts, and care coordination in Epic and other required documentation.
• Maintain professional boundaries and confidentiality while collaborating with patients, families, and cross-functional teams.

Educación
