Job summary

Community Bridges, Inc. seeks a Navigator I to enhance client success by identifying barriers and connecting individuals to needed services. This role conducts outreach, engagement, case management, and wrap-around supports, documenting interactions per AAC and DBHS guidelines as part of a multidisciplinary team.

The Navigator I supports clients with care navigation, service coordination, and next-step planning to achieve ongoing success within Arizona's behavioral health system.

Qualifications

• Highschool diploma or GED required; an Associate degree in a behavioral health field is preferred.
• Minimum six months recovery from substance use or mental health disorders required.
• 1–3 years health care-related work experience is preferred but not required.
• 1–3 years of recovery-oriented service is preferred but not required.
• Clear 39-Month Motor Vehicle Record.
• Arizona Fingerprint Clearance required.

Responsibilities

• Engage clients and conduct outreach, engagement, and wrap-around supportive services.
• Map out next steps to access services and document interactions per AAC/ADHS-DBHS guidelines.
• Work within a multi-disciplinary team and track required patient interactions.
