Job summary

Community Bridges, Inc. (CBI) is seeking a Navigator I to engage clients, identify barriers, and connect them with necessary services.

You will perform outreach, case management, and wraparound supports as part of a multidisciplinary team, documenting interactions in line with AAC, ADHS/DBHS, CARF, and CBI policies. The Navigator I role focuses on mapping the best next steps for clients in crisis or needing ongoing support, with emphasis on recovery and service coordination across behavioral

Qualifications

• Highschool diploma or GED is required.
• Associates Degree (or higher) in Behavioral Health is preferred but not required.
• Minimum six months of recovery from substance use and/or mental health disorders is preferred.
• 1-3 years of health care related work experience is preferred.
• 1-3 years of substance abuse/mental health recovery-oriented service is preferred.
• Driver's License in good standing.
• Clear 39-month Motor Vehicle Record.

Responsibilities

• Enhance the likelihood of client success and reduce barriers to services.
• Conduct outreach, engagement, case management, and wraparound supportive services.
• Map out next steps for individuals needing access to services and resources.
• Document and track required patient interactions per AAC, ADHS/DBHS, CARF, and internal procedures.

Education
