Job summary

Community Bridges, Inc. (CBI) seeks a Navigator I to support individuals with behavioral health needs across Arizona. You will conduct outreach, engagement, case management, and wrap-around services to reduce barriers and link clients to resources. Join a multidisciplinary team and document interactions per state and CARF guidelines.

Qualifications

• Highschool diploma or GED required; associates degree preferred.
• Minimum six months recovery from substance use and/or mental health disorders.
• 1–3 years of health care related work experience preferred.
• 1–3 years of recovery-oriented service preferred.
• Valid AZ driver's license and clean 39-month MVR.

Responsibilities

• Conduct outreach, engagement, case management, and wrap-around services.
• Map out next steps to access services and resources.
• Document and track patient interactions per AAC/ADHS/DBHS/CARF requirements.
