Job summary

Community Bridges, Inc. is seeking a Navigator II to support clients by identifying barriers and connecting them with needed services. You will conduct outreach, engagement, and ongoing case management to help individuals access housing, healthcare, and recovery supports.

As part of a multidisciplinary team, you will document interactions per AAC/ADHS/DBHS guidelines and coordinate wraparound services to achieve lasting outcomes for clients across Arizona.

Qualifications

• High school diploma or GED required; associate degree preferred.
• Minimum 12 months of recovery from substance use and/or mental health disorders.
• 1–3 years of health care related work experience preferred but not required.
• 1–3 years of substance abuse/mental health recovery-oriented service preferred but not required.
• Minimum of 90 days experience in a Navigator or Peer Support Specialist role.
• Arizona Driver's License in good standing; must provide a 39-month MVR.
• Arizona Fingerprint Clearance card (program specific).

Responsibilities

• Enhance likelihood of client success by identifying barriers and connecting to needed services.
• Conduct outreach, engagement, and ongoing wraparound supportive services.
• Map out next steps for clients and document interactions per AAC/ADHS/DBHS/CARF policies.
