Job summary

Community Bridges, Inc. is seeking a Navigator II to enhance client engagement, identify barriers, and connect individuals with essential services. You will conduct outreach, perform case management, and coordinate wrap-around support within a multi-disciplinary team.

The role requires a high school diploma or GED, 6 months of recovery experience, and driver’s license. FP clearance and MVR verification are required as part of program policies.

Qualifications

• High school diploma or GED required.
• Six months of recovery from substance use and/or mental health disorders required.
• 1–3 years of healthcare-related work experience preferred.
• At least 21 years old with a valid driver’s license.
• Fingerprint clearance card required and 39-month MVR.
• Must be able to provide documentation per program policies.

Responsibilities

• Engage and outreach to clients to identify needs and barriers.
• Coordinate access to housing, health, treatment, and community services.
• Provide ongoing case management and wrap-around supports.
• Document client interactions per AAC/ADHS-DBHS/CARF and internal policies.
