Job summary

Community Bridges, Inc. seeks a Navigator II to support clients by outreach, engagement, case management, and wraparound services. You will collaborate with a multi-disciplinary team to identify needs and connect individuals to housing, treatment, and social supports.

Ideal candidates have a current AZ Driver's license, valid fingerprint clearance, and at least 12 months in recovery-focused care; 21+ and strong communication are essential for success in this role.

Qualifications

• Highschool diploma or GED required; related associate degree preferred.
• Minimum 12 months recovery from substance use and/or mental health disorders.
• 21+ years old; AZ Fingerprint Clearance card; valid AZ driver’s license; 39-month MVR.

Responsibilities

• Conduct outreach and engagement with individuals in need of services.
• Provide case management and wraparound supportive services.
• Document required patient interactions per AAC, ADHS/DBHS, CARF and internal procedures.
