Job summary

Community Bridges, Inc. (CBI) seeks a Navigator II to support clients by conducting outreach, engagement, and comprehensive case management within a multi-disciplinary team.

The role focuses on identifying barriers to services, mapping next steps, and coordinating wrap-around supports to promote sustained engagement and recovery for individuals with behavioral health and substance use needs.

Qualifications

• Highschool diploma or GED required; an associate degree in behavioral health is preferred.
• Minimum of 12 months of recovery from substance use and/or mental health disorders required.
• 1-3 years of health care related work experience may be preferred but not required.
• 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
• Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
• Must possess a current valid driver's license. Must be able to provide a 39-month MVR.
• Arizona Fingerprint Clearance card (program specific).
• Current AZ Driver's License (Valid and in good standing).
• Clear 39-month Motor Vehicle Record.

Responsibilities

• The Navigator II's primary role is to enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources.
• The Navigator II will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services.
• The Navigator II will document, and track required patient interactions according to the AAC, ADHS/DBHS/ CARF, and Community Bridges Policies and Procedures.
