Community Bridges, Inc. (CBI) seeks a Navigator II to support individuals in need of additional services across Arizona. You will conduct outreach, engage clients, provide case management, and coordinate wraparound supports to reduce barriers and promote ongoing success.

You'll work within a multi-disciplinary team and document interactions according to AAC, ADHS/DBHS, CARF, and internal policies, ensuring compliance while helping clients access housing, treatment, and community resources.

Qualifications

• Highschool diploma or GED required.
• Associate degree (or higher) in Behavioral Health preferred.
• Minimum of six months recovery from substance use and/or mental health disorders required.
• 1-3 years of full-time health care related work experience preferred.
• 1-3 years of recovery-oriented service preferred.
• Minimum of 90 days experience as Navigator or Peer Support Specialist required.
• Valid driver’s license.
• Clear 39-month Motor Vehicle Record.
• Arizona Fingerprint Clearance Card.

Responsabilités

• Enhance likelihood of success by identifying barriers and linking clients to services.
• Conduct outreach, engagement, case management, and wraparound support.
• Map next steps for individuals needing assistance accessing services.
• Document patient interactions per AAC, ADHS/DBHS, CARF, Community Bridges Policies and Procedures.
