Job summary

Community Bridges, Inc. seeks a Navigator II to enhance client outcomes by outreach, engagement, and coordinating wrap-around services. You will work with a multi-disciplinary team and map steps to access needed resources for ongoing success.

The role requires documenting interactions per relevant regulatory and program guidelines, ensuring compliance and quality care for clients in Arizona.

Qualifications

• Highschool diploma or GED required.
• Associates degree (or higher) in Behavioral Health preferred but not required.
• Minimum six months recovery from substance use and/or mental health disorders.

Responsibilities

• Conduct outreach, engagement, case management, and wrap-around services.
• Map out next steps to access needed services for ongoing success.
• Document patient interactions per AAC/ADHS-DBHS/CARF policies.

Education
