Project HOME is seeking a Residential Services Coordinator 2 to provide person-centered case management and advocacy for residents at Ruth Williams House in Philadelphia. You will support housing stability, wellness, and recovery goals in a compassionate, LGBTQ+-affirming environment.

This full-time role involves coordinating housing, healthcare, and community resources, maintaining documentation, and participating in on-call rotations.

Residential Services Coordinator 2 - RWH

As a Residential Services Coordinator (Case Manager), you'll support individuals on their recovery journey by providing person-centered case management, resource coordination, and advocacy in an LGBTQ+-affirming supportive housing environment.

Working alongside residents, you'll help individuals achieve their housing, employment, wellness, and recovery goals while creating a community built on dignity, respect, and hope. If you're passionate about helping people overcome barriers and build brighter futures, we'd love to hear from you.

• Location: Ruth Williams House - 2415 N. Broad Street, Philadelphia, PA 19132
• Schedule: FullTime | Monday through Friday, 10AM to 6PM
• Compensation: Up to $23.90/hour, based on experience

What You'll Do

Support Resident Recovery

• Build trusting relationships and provide person-centered support.
• Partner with residents to develop and achieve individualized recovery goals.
• Respond to crises using appropriate intervention and de-escalation techniques.
• Promote independence, wellness, and housing stability.

Coordinate Services

• Connect residents with housing, healthcare, behavioral health, employment, education, and community resources.
• Assist with housing applications, documentation, and discharge planning.
• Collaborate with internal teams and community partners to coordinate care.

Foster Community

• Facilitate recovery-focused groups and community-building activities.
• Create a safe, welcoming, and LGBTQ+-affirming environment.
• Encourage resident engagement, peer support, and skill development.

Documentation & Program Support

• Maintain timely and accurate case documentation.
• Participate in team meetings, training, and an on-call rotation.
• Provide program support and perform site-specific duties, including meal preparation and medication monitoring, as needed.

What You'll Need

Education

One of the following is required:

• High school diploma or GED and four (4) years of related experience
• Associate degree and two (2) years of related experience

Experience

• Experience supporting individuals experiencing homelessness, behavioral health challenges, substance use disorders, or other vulnerable populations.
• Availability to work evenings, weekends, and participate in an on-call rotation.

Knowledge, Skills & Abilities

• Strong verbal, written, and interpersonal communication skills.
• Strong organizational and time management skills.
• Computer proficiency and the ability to learn electronic case management systems.
• Ability to build trusting relationships while maintaining professional boundaries.
• Commitment to person-centered, recovery-oriented care.

What You May Bring

While not required, the following qualifications may help you succeed in this role:

• Experience providing case management in supportive housing, residential services, or community-based programs.
• Knowledge of trauma-informed care, recovery-oriented practices, or harm reduction.
• Experience facilitating groups, workshops, or life-skills programming.
• Familiarity with housing resources, public benefits, and community-based support services.
• Experience working with individuals experiencing homelessness, behavioral health challenges, or substance use disorders.
• Experience working in an LGBTQ+-affirming environment.
