Job summary

Project HOME is seeking a Residential Services Coordinator II to provide direct support to residents in supportive housing programs. The role combines case management, resource coordination, crisis intervention, and recovery planning to help residents achieve housing stability and wellness.

The ideal candidate brings person-centered care, inclusive community focus, and a passion for helping individuals rebuild their lives.

Qualifications

• Experience working with individuals experiencing homelessness and/or behavioral health challenges.
• Experience in case management, residential services, recovery support, or human services.
• Availability to work evenings, weekends, and participate in an on-call rotation.

Responsibilities

• Build trusting relationships with residents and support their transition from homelessness to stable housing.
• Collaborate with residents to identify strengths, needs, and goals.
• Develop individualized recovery plans with both short-term and long-term objectives.
• Respond to crisis situations using appropriate intervention and de-escalation techniques.
• Foster a culture of recovery, wellness, dignity, and peer support.
• Connect residents to housing, employment, income, education, benefits, healthcare, and behavioral health resources.
• Assist residents with accessing community supports that promote independence and stability.
• Advocate on behalf of residents and help remove barriers to success.
• Support residents in obtaining documentation and navigating service systems.
