HealthRIGHT 360 is a nonprofit delivering integrated healthcare, mental health services, and substance use treatment in San Francisco. The Residential Case Manager coordinates care for clients in 815 Residential and Detox, linking them to medical, behavioral health, housing, and community resources to support recovery and reintegration.

Qualifikationen

• High school diploma or equivalent required.
• Bachelor's degree preferred.
• AOD Registration from an Accredited authority (CCAPP, CADTP, CAADE) is required.
• AOD Certification from an Accredited authority (CCAPP, CADTP, CAADE) is preferred.
• First Aid and CPR certification.
• A valid California driver’s license.

Aufgaben

• Conduct comprehensive assessment of case management needs using ASAM criteria.
• Coordinate benefits, including Medi-Cal, and develop individualized care plans.
• Link clients to medical, mental health, housing, and community resources.
• Develop aftercare and discharge plans and coordinate transitions.
• Maintain timely documentation per DMC-ODS guidelines.
• Collaborate with treatment teams and provide crisis management as needed.
