Job summary

Attain Behavioral Health is seeking a compassionate Case Manager to support clients through substance use disorder treatment and recovery. You will identify barriers, coordinate services, and connect clients with housing, employment, healthcare, and community resources.

Qualifications

• Must have high school diploma or equivalent.
• CDCA credential required.
• QMHS III qualification or ability to meet requirements.
• Minimum 1 year of relevant behavioral health or case management experience.
• Working knowledge of community resources and social-services systems.
• Strong crisis intervention and de-escalation skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide case management services to clients in substance use disorder and behavioral health programs.
• Assess client needs and barriers across housing, employment, transportation, healthcare, mental health, legal, benefits, and education.
• Develop and maintain individualized case management/service plans.
• Provide ongoing support and service coordination to meet goals.
• Connect clients with housing, employment, medical, transportation, and community resources.
• Coordinate care with therapists, peer supporters, medical providers, and other team members.
• Utilize crisis intervention and de-escalation techniques as needed.
• Maintain professional, calm, and ethical client-centered interactions.
• Maintain timely, confidential documentation per policies and regulations.
• Participate in interdisciplinary team meetings, reviews, and trainings.
• Keep current knowledge of resources and regulatory requirements.
