Job summary

ReDiscover’s Transitions Program provides medication-assisted treatment (MAT) for individuals with opioid use disorder, offering methadone and buprenorphine along with counseling, group therapy, case management, and integrated mental health and nursing services.

As a Community Support Specialist, you will manage a caseload, connect clients to housing, employment, and other supports, coordinate care and transportation, and document progress in the Carelogic EHR to support positive treatment

Qualifications

• Bachelor’s degree or 4+ years of equivalent experience.
• Two+ years of experience in a substance use, behavioral health, or related setting preferred.
• Knowledge of Kansas City metro area community resources and health/social service systems.
• Strong ability to build relationships and engage clients in a recovery-oriented environment.
• Excellent written, verbal, and interpersonal communication skills.
• Proficiency in Microsoft Office and general computer skills.

Responsibilities

• Manage and actively engage a caseload of clients in treatment for substance use disorders.
• Support clients in progressing through recovery and relapse prevention strategies.
• Build trust with hard-to-engage clients to promote consistent participation in treatment services.
• Help reduce barriers by connecting clients to housing, employment, education, and other supports.
• Coordinate care and provide transportation using your personal vehicle as needed per treatment plans.
• Document client progress and interactions in the Carelogic EHR; collaborate with the multidisciplinary team.
