Job summary

PCH Princeton Community Hospital in Bluefield, WV seeks a Recovery Coach to guide individuals with substance use disorders identified in WVU Medicine affiliated settings. You will help link patients to outpatient treatment, develop and monitor individualized plans, and provide ongoing support to stay engaged in care.

Qualifications

• High school diploma or equivalent required.
• Completion of PRSS-SUD or CPRS certification within specified timeframes.
• State certification within one year where services are provided.

Responsibilities

• Assist patients with identifying treatment needs and developing individualized plans.
• Provide mentorship and life-experience guidance to individuals with substance use problems.
• Meet patients face-to-face or by phone as agreed, to review wellness plans.
• Share information with the treatment team to inform decision-making.
• Link patients to housing, education, recovery supports, and health services.
• Serve as liaison to supports including families, sponsors, and providers.
• Maintain relationships with WVU Medicine staff and external agencies.
• Assist patients in crisis situations involving social, emotional, or health problems.
