Qualifications

Responsibilities

• Acts as peer support during hospitalization and post-discharge, assisting patients and families in accessing community services.
• Completes required documentation and other reports; notifies nurse before visiting the patient.
• Conducts brief interventions for patients who screen positive for alcohol/drug use; collaborates with nursing and social work staff.
• Assesses patient history of substance use and readiness to change; uses motivational interviewing techniques to support a recovery plan.
• Provides family members with recovery materials and conducts outreach to ensure follow-up and linkage to services.
