Job summary

PCH Princeton Community Hospital is seeking a Recovery Coach to serve as a liaison for individuals with substance use disorders in WVU Medicine affiliated settings. You will help link patients to outpatient treatment, develop individualized wellness plans, and support ongoing recovery within a care team.

Responsibilities include mentoring, meeting with patients and support persons, coordinating resources, and participating in crisis intervention as needed.

Qualifications

• High School diploma or equivalent required.
• Completion of state or nationally recognized Peer Recovery Support Specialist Certification training within 90 days of hire.
• Peer Recovery Certification issued by state board within one year of hire.

Responsibilities

• Work actively with patients with Substance Use Disorders identified in WVU Medicine treatment settings.
• Provide mentoring through life-experience to individuals experiencing substance use problems.
• Meet face to face with individuals and identified support persons in person and/or by phone, at a frequency and format requested and agreed upon by the individual.
• Develop individualized plans with patients and monitor those wellness plans during meetings.
• Provide information to treatment team to inform treatment decision making.
• Link patients to available resources, including continuing education/professional development, safe housing, recovery supports, self-help groups, substance abuse and mental health services, sobriety meetings, and medical/health facilities.
• Serve as a liaison to the patient’s formal and informal supports, including referral sources, family members, client sponsors, treatment providers, and other supporters.
• Maintain working relationships with WVU Medicine treatment staff and other agencies or institutions in order provide awareness of recovery as a reality and the availability of recovery coaching.
• Assist patients in crisis situations involving social, emotional, health, and other problems.
