Job summary

Charleston Area Medical Center in Grafton Hospital is seeking a Peer Recovery Coach to provide non-clinical support to patients with high-risk alcohol and drug use, both in the hospital and in the community.

You will work with the hospital team to motivate change, develop service plans that promote linkage to treatment and recovery resources, conduct outreach after discharge, and provide ongoing peer support to help patients achieve personal recovery goals.

Qualifications

• Requires High School Diploma or GED.
• Experience in client-facing roles preferred.
• Recovery-focused training or peer support experience preferred.

Responsibilities

• Conducts brief interventions for patients that screen positive for risky alcohol and/or drug use.
• Identifies patient’s readiness to change and motivates using motivational interviewing.
• Coordinates with hospital team to link to community services and resources.
• Provides ongoing peer support during hospitalization and post-discharge.
• Assists patients in accessing transportation and scheduling appointments.
• Educates patients and families about naloxone and overdose risks.
