Job summary

Delta-T Group is seeking a Recovery Case Manager in the Kensington area of Philadelphia. The position is 85% hybrid or community-based, requiring a valid driver’s license and access to a vehicle. Candidates must have lived experience with homelessness and substance dependency, and be prepared to work with participants facing D&A challenges.

Qualifications

• Bachelor's degree in Human Services or related field.
• 1+ year experience as case manager.
• Lived experience with homelessness and dependency (drug & alcohol abuse).
• Ability to engage with adults dealing with addictions.

Responsibilities

• Collaborate with participants to formulate and review a comprehensive service plan.
• Provide outreach, service coordination, counseling, advocacy, and other needed services in various environments (streets, shelters, prisons, hospitals, homes, offices).
• Deliver consumer-driven services such as housing placement, independent living skills, vocational guidance, and appointment escorts.
• Conduct regular home visits and accompany clients to medical appointments and treatment facilities as appropriate.
• Perform other duties as assigned.
